how to dispute when cheated by a benefits vendor?

i got enrolled in a monthly commuter benefit payroll deduction (tax free) scheme last yr. I soon realized that i didnt need one and faxed the cancellation form to the vendor, but when i checked my payslips i still see deductions being made. When i questioned the vendor, they simply denied having received the cancellation form, nor do i have saved any e-mail conversations or fax confirmation receipt with me.

How should I dispute?

Threaten them by stating that you will take legal action if they do not stop the deductions and reimburse for retro funds starting from the date you requested the cancellation.

If no action is taken by them, file with the county and take them to small claims court.
